The Financial Counselor provides assistance to patients with Hospital Presumptive Eligibility (HPE) and Charity Care applications; may act as a liaison between the patient and Patient Advocate Vendor to obtain MediCal. Performs a variety of procedures to assist in providing laboratory services. Follow established policies and protocols including but not limited to handling and accessioning specimens, entering laboratory information into computer, and instructing patients on specimen collection.  Provides clerical and customer service support to laboratory department.

Performs analysis of inpatient and outpatient records to assure their completeness. Assigns physician deficiencies in a timely manner. Monitors incomplete records, provides notification to physicians of delinquencies and performs physician suspension when required. This position is responsible for the daily work and work-flow within the inpatient units (Medical/Surgical and ICU). The Inpatient Clinical Coordinator (ICC) is a Registered Nurse who works with the Director of Patient Care Services to ensure operational and hospital standards are upheld. The ICC is integrally involved in ensuring that staff, patients, and other department services come together seamlessly to advance patients through their admitted stay and ensure that the patients/caregivers have adequate knowledge of goals of care and discharge plans. Contributes to a nurturing and healthy work environment for all team members, ensuring the highest quality of care is delivered.

Under the direction of the Patient Access Manager the Patient Access Representative is responsible for interviewing patients or their representatives in order to determine and record all demographic and financial information necessary to accurately and effectively process the registration, billing and payment functions. The Patient Access Representative will perform these services within the context of Sonoma Valley Hospital's mission, goals and objectives. 

The X-Ray/CT Technologist performs all radiologic (x-ray and computed tomography) examinations and techniques while obtaining complete, high quality, diagnostic examinations for Radiologists. Administers contrast as prescribed. Assists physicians during procedures, adhering to sterile technique guidelines. Monitors and maintains patient safety at all times. Performs routine quality control on x-ray and CT equipment.

The Echocardiograph Technician performs echocardiography examinations according to department procedure as required for age-specific populations. The technician will provide preliminary exam findings to the Cardiologists to aid in the interpretation of the study. Ensures that all exams are documented accurately.

Under the direction of the Supervising Physician provides primary care in the physician clinic. Duties include providing general and preventative care, educating patients about preventative care and prescribed treatments. Serves as a clinical resource to medical assistants, reviews and revises clinical policies and procedures per policy.

The Rehab Clinical Coordinator assists the Director of Rehabilitation Services in the daily operations of the Rehab dept including staffing and quality improvement. Evaluates and treats patients in both the inpatient and outpatient physical therapy setting. Provides accurate and timely documentation and coordinates care with other health care professionals. Will help to develop and prepare monthly productivity and quality reports. Participates in community outreach and serves as a resource in community education.

Performs a variety of roles to assist in the care of patients in the operating room in accordance with established policies and protocols for surgical technologists. Strong ortopedic experence preferred.

The Physical Therapist Assistant treats patients primarily in the outpatient physical therapy setting following an established plan set by the physical therapist. Provides advice and education to patient, family and caregivers. Regularly assesses patients clinical signs and symptoms to determine effectiveness of treatment as well as need to modify goals or treatment or plan discharge in conjunction with physical therapist. Provides accurate and timely documentation and coordinates care with other health care professionals. Effectively participates in clinical site education through in-services and clinical education of assigned students.

The Registered Nurse (RN) is an integral part of the patient care team. The RN assists in the delivery of high quality, individualized patient care. The RN autonomously practices within the scope of RN licensure as defined by the California Board of Registered Nursing, and the Nurse Practice Act. The RN advocates for patients and their families, the care team and the organization.  The RN designs and carries out the plan of care utilizing the nursing process of assessment, diagnosing, planning, implementing and evaluation.

The Certified Nursing Assistant (CNA) is an important member of the patient care team, supporting patients, nurses and also overall unit functioning. The CNA is responsible for helping create and maintain a safe, healing environment for our patients, and a healthy working environment for the care team.  The CNA is expected to perform duties autonomously and also at the direction of the RN. The CNA reports to the Director of Patient Services.

The Physical Therapist - Inpatient & Outpatient Hybrid position evaluates and treats patients in both the acute inpatient hospital physical therapy setting as well as in the Outpatient Hand and Physical Therapy clinic. Provides accurate and timely documentation and coordinates care with other health care professionals. In the hospital setting, will work closely with the Hospitalists, Nursing, Case Management staff and families/caregivers. In the Oupatient setting will work closely with referral sources and families/caregivers as well as physical therapy assistants. Effectively participates in clinical site education through in-services and clinical education of assigned students.

The  Physical Therapist evaluates and treats patients primarily in the hospital physical therapy setting. Provides accurate and timely documentation and coordinates care with other health care professionals. The dietitian works closely with the inpatient and outpatient multidisciplinary healthcare team to provide patients/residents care that is integrated and compatible with the patient/resident focused medical goals and objectives.
