Under the direction of the Patient Access Manager the Patient Access Representative is responsible for interviewing patients or their representatives in order to determine and record all demographic and financial information necessary to accurately and effectively process the registration, billing and payment functions. The Patient Access Representative will perform these services within the context of Sonoma Valley Hospital's mission, goals and objectives. 

The Emergency Nurse is responsible for high quality individualized patient care in the Emergency Department requiring a broad range of knowledge and expertise in the treatment of patients experiencing medical emergencies ranging from low acuity (earaches) to full blown resuscitative encounters (acute stroke, cardiac arrest).  Practices within the scope of RN licensure as defined by California Board of Registered Nursing, the Nurse Practice Act and Emergency Nurses Association standards of practice.  Advocates for and participates in high quality patient care and patient experience.

The Director of Quality provides direct oversight of Quality Management, Risk Management, Infection Prevention, and Case Management. Under the direction of the Medical Director provides primary care in the physician clinic. Duties include providing general and preventative care, educating patients about preventative care and prescribed treatments. Serves as a clinical resource to medical assistants, reviews and revises clinical policies and procedures per policy. 

Provides social services to patients with identified social, behavioral and/or mental-emotional needs in the acute and emergency department settings. Acts as a liaison with community provider agencies to develop care resource networks. Works with the multidisciplinary team to build effective care coordination planning for patients and their families.

The Case Manager leads care coordination throughout hospital levels of care. In this role the Case Manager completes assessments  and transition care planning that reflects the wishes and needs of the patient and their family. This position also assists physicians and finance with the management of  resource utilization through the use of medical necessity criteria.

The Registered Nurse (RN) is an integral part of the patient care team. The RN assists in the delivery of high quality, individualized patient care. The RN autonomously practices within the scope of RN licensure as defined by the California Board of Registered Nursing, and the Nurse Practice Act. The RN advocates for patients and their families, the care team and the organization.  The RN designs and carries out the plan of care utilizing the nursing process of assessment, diagnosing, planning, implementing and evaluation.

They provide nursing care from assessment to outcome evaluation. With this role, the RN Circulator is usually the single advocate for patient safety during operative procedures. The RN Circulator assists the team in creating and maintaining a comfortable and safe environment for the patient and observes the team from a wide perspective to provide safety within the period of the operation.

The RT is an important member of the patient care team, providing respiratory care, support, and teaching of pediatric and adults patients in the ED, MS, ICU, and PACU.   The RT is responsible for helping create and maintain a safe, healing environment for our patients, and a healthy working environment for the entire care team.  The RT is expected to perform duties autonomously. The RT reports to the Director of Patient Services.  

SCU (Surgical Care Unit) RN is responsible for the preoperative preparation and recovering of all surgical patients. Responsible to act as an integral team member and keep patients moving through the unit in a safe, efficient, and timely manner. Able to apply critical thinking skills throughput the surgical continuum for each patient under their care. Skilled in post anesthesia recovery care of patient undergoing various types of procedures with different combinations of anesthesia.

The Speech Therapist administers diagnostic speech, language, cognitive-linguistic and swallowing evaluations for adult and pediatric populations and provides therapeutic intervention. Prepares timely, complete documentation. Attends interdisciplinary care planning meetings.

The  Physical Therapist evaluates and treats patients primarily in the hospital physical therapy setting. Provides accurate and timely documentation and coordinates care with other health care professionals. Effectively participates in clinical site education through in-services and clinical education of assigned students. 

The Financial Counselor provides assistance to patients with Hospital Presumptive Eligibility (HPE) and Charity Care applications; may act as a liaison between the patient and Patient Advocate Vendor to obtain MediCal. Works though patient's financial obligations, both with and without insurance, and provides assistance with payment arrangements. Responsible for the collection and follow-up of accounts with patient liability in accordance with collection guidelines and facilitates the timely receipt of patient liabilities for hospital services.
